You will need to provide details of your GP practice name, your name, your date of birth, medicines required and your usual pharmacy. Your prescripion will be ready for collection after one week.
Remember if you can’t make it – PLEASE cancel your appointment.

The receptionist will need to take some details about the problem and arrangements will be made for you to be seen by the doctor as soon as possible. Please see our Urgent Problems and Out of Hours page for information.
Please try to ring for routine appointments between 8am & 5pm. Sometimes problems and reviews can be dealt with over the phone via a booked telephone appointment which the receptionists can book you in for. This can save you the need to attend the surgery to be seen in person.
GP appointments in the practice last for about 10 minutes, so please don't present more than 2 different problems per consultation.
A single consultation is for one person only. Please do not expect the doctor to see more than this; they will probably ask the patient without an appointment to come back another time.
Phone consultations last for 5 minutes and only 1 problem can be dealt with in that time.

If you are late for your practice appointment, you will either have to wait until the end of surgery to be seen or re-book for another day. If someone has canceled their appointment during the surgery you may take their vacant slot. If you turn up late for your appointment after the surgery has finished (without telling us first) you will not be seen except in exceptional circumstances and you will have to book another appointment.
If you feel that your problem will take longer than 10 minutes for the doctor to sort out, please mention this when you book your appointment as a longer consultation can be arranged.
This service reminds you of an appointment via a text message. All registered patients are automatically opted into the service.
Please ensure if you wish to use this service we have your correct mobile telephone number and inform us as soon as possible if your number changes.
If you do not wish to subscribe please contact the surgery and inform reception that you wish to opt out.
If you anticipate that you may require an examination by the Doctor and feel that you would like a chaperone to be present, please make reception aware at the time of booking your appointment so that this can be arranged. For further information please see our Chaperones policy.
If you are unable to attend for your appointment please tell us in good time so that we are able to offer another patient the use of that appointment.

As part of our commitment to all our patients we monitor the efficient use of appointments. If a patient regularly fails to attend a booked appointment, the practice reserves the right to remove them and their family from the practice list.
