PATIENT REPRESENTATIVE GROUP:

Are you interested in finding out more about Moredon Medical Centre and the health care environment as a whole?
Would you like to influence the development of local services?
We are keen to set up a Patient Representative Group, and we need you to help us. If you are a patient of the Moredon Medical Centre and wish to join this representative group by giving us your e-mail address, periodically we will ask for your input into a number of surveys via the practice website, which will allow us to develop and improve services to meet the needs of our patients.
To join the Group you will need an e-mail address and Internet access. If you are interested please download and complete this form, then email it to moredonmedicalcentre@nhs.net
We hope that the group will be reflective of the surgery patients and will involve many people of different ages and backgrounds.
